Switches the label taxonomy to an all-/-scoped scheme and makes the agent/* command scope exclusive — only one agent runs per issue at a time (no parallel agents); summoning one stage auto-clears the previous.

Changes

labels.json: agent:* → agent/* (exclusive); plan:approved → plan/approved; ai:off → ai/off. Legacy plan:ready and pr:open removed. 28 labels total.
Agent defs + agents/README.md: summon/sets_labels and prose updated to the new names; dropped plan:ready/pr:open; Conventions rewritten (all-/, agent/* exclusive = one agent per issue).
agents/triage.md: fixed the incoherent identity — now runs as claude-plan (planner, can comment + set tier/*) instead of the read-only agent-read.
dispatch.yaml + repo README.md: route conditions and diagram updated to agent/plan / plan/approved so the live plan + implement agents keep firing under the new names.

Follow-ups (not in this PR)

Seed the new labels into each org: ADMIN_PAT=… ./seed-org-labels.sh valssi valssi-apps valssi-ai valssi-agents valssi-public
Delete the old :-scheme labels from valssi/ops + the 4 legacy valssi org labels.

Merge is human-only per policy.
